It's really looking at institutes dealing with the insane. Many of the people in those cells don't think they have a problem, and they don't think their broken. The reality of things are that they are broken.

Once poison enters your bloodstream, it doesn’t leave. It stays in your body till you die, pumping around your body non-stop. The only relief that people find is that they know the poison will exit their body eventually, the downside of this, i that it will only exit once you’re dead.
